11. Timedsurgical mixed peeling of the lower eyelids

Timedsurgical mixed peeling is the technique of choice for the permanent elimination of lip wrinkles, even if they are deep. Mixed peeling can also be used to eliminate the effects of ageing on the skin of the lower eyelids; these effects may range from slight roughness to evident wrinkles and large folds, as in this patient.
The effects of skin ageing in the palpebral region are difficult to correct by means of surgical techniques. However, if the overall appearance of the face is to be successfully rejuvenated, skin ageing of the eyelids needs to be treated. Just think of a face that has been rejuvenated by means of elastic lifting, but which still has aged skin in the palpebral regions. Poor quality of the palpebral skin can even spoil the result of perfectly performed surgical blepharoplasty. Indeed, to be effective, facial rejuvenation must be complete and must therefore also involve the skin of the eyelids.

Benign neoformations of the edge of the eyelid can easily be removed by means of slow pulsed timedsurgical cutting (Timed, Korpo SRL, Italy). Electroshaving is one of the techniques of Timedsurgery (Technique for Implementation of Misured Electrosurgical date ).
Electroshaving allows optimal visibility, good haemostasis and perfect control of the excision. Moreover, it does not cause burning of the edges. Timedsurgical electroshaving is the technique of choice for the removal of benign neoformations. This method exploits the regenerative capacity of the organism.

This patient is to undergo upper and trans-conjunctival blepharoplasty by means of rapid pulsed timedsurgical cutting. Her malar pouches will be corrected by means of timedsurgical Adipolysis.
A partially insulated electrode inserted through the skin will eliminate the pouch by vaporizing the superficial and deep adipose cells.
Before the procedure, the operator draws lines on the skin to indicate the palpebral skin to be removed and the area where the malar pouch is present.
Local anesthesia is carried out by means of equal amounts of 2% mepivacaine and 2% lidocaine. We add ½ mg – 1 mg of epinephrine to the 10 ml vial of mepivacaine.

08. Treatment of xanthelasma with timedsurgical resurfacing

The lower eyelids of this young patient present extensive xanthelasmas. Timedsurgical resurfacing is the technique of choice for eliminating xanthelasmas, even those which recur. What you see here is one of the more than 72 techniques of Timedsurgery. Local anaesthesia is carried out by means of mepivacaine with epinephrine. The xanthelasmas will be eliminated by means of timedsurgical resurfacing, which does not heat the deep tissues. Treatment is normally carried out in two sessions. During the second session, which is held two months after the first, a magnifying glass is used to check whether any residual areas of xanthelasma are still visible. If they are, they are treated by means of the same program data used in the first session. The Timed apparatus is set to the resurfacing function in the coagulation mode and the power is set to 38 or 50 W. An EM 15 electromaniple is mounted. The tip of the electromaniple, which has a diameter of 1.5 mm, must be placed very close to the surface of the skin in order to generate the micro-arcs that vaporise and dry the lesion. The electromaniple must be kept moving at all times. The operator carries out several sweeps in the areas where the xanthelasma is thickest. Resurfacing must also be carried out a few mm around the visible lesion.

07. Permanent elimination of lip wrinkles by means of pulsed timedsurgical de-epithelialisation and the application of resorcin

Lip wrinkles can be eliminated easily and permanently by means of mixed timedsurgical peeling. This consists of pulsed timedsurgical de-epithelialisation followed by the application of a saturated solution of resorcin for one to two minutes. Mixed timedsurgical peeling is the method of choice even for the elimination of deep lip wrinkles. A single treatment session is sufficient in order to obtain a permanent result. This ambulatory procedure is carried out under loco-regional anaesthesia.
Once the Timed apparatus has been set to the pulsed de-epithelialisation function, in the coagulation mode at a power of 2 Watt, the lip is moistened with a saline solution. The procedure is carried out by skimming the skin with the edge of the angle of a 0,20 mm diameter EM 10 Yellow electromaniple. The specific high-frequency current lifts the epidermis from the dermis while maintaining the integrity of the dermal papillae and the capillary-papillary plexus. De-epithelialisation is started 5 mm externally from the naso-labial fold.

Dermal-epidermal patches on the face can easily be removed by means of timedsurgical mixed peeling. This consists of pulsed timedsurgical de-epithelialisation of the skin and the subsequent application of a saturated solution of resorcin in water for 20 seconds. Treatment is carried out under local anaesthesia. De-epithelialisation is performed with the edge of the angle of the EM10 Yellow electromaniple at a power of 2 Watts in the pulsed coagulation function specifically designed for de-epithelialisation (Direct Pulsed, 4/9 hundredths of a second, Coag, 2 Watts, EM 10 Yellow (bent at an angle). As the electrode skims the surface of the patch in small movements, the epidermis detaches from the dermis. Timedsurgical de-epithelialisation is carried out on the entire visible surface of the patch, extending to about 1 mm beyond it. Electrical conductivity can be improved by moistening the skin with physiological solution.
