This patient underwent liposuction for a double chin and, at the same time, an Elastic Neck Lift with implantation of an elastic thread between the two fasciae of Loré. A year later, we are correcting the folds that form in the lower part of the neck when the patient bends her head forward. No skin is removed during this procedure; the tissues are simply repositioned. The excess cutaneous and subcutaneous tissue that is moved sideways and upwards will not only remain invisible, but will also serve to counteract wasting of the sides of the neck. Upward and lateral traction is applied to the skin of the neck with the fingers, in order to identify the best position for the oblique suspensive vectors that will be used to lift the excess skin in the infero-anterior region of the neck.
A) Stabilization of surgical mastopexy, with volume enhancement of the upper pole and conization of the breasts.
The patient we see here has markedly drooping breasts. We are going to perform vertical mastopexy and implant a stabilizing Elasticum EP4 suspension thread, which is anchored to the subcutaneous tissue of the chest, and an elastic conizing thread. The only dissection carried out is underneath; the subcutaneous tissue is removed in order to shape the lower area, where the vertical incision ends.
The elastic thread is implanted at a depth of about 1.5 cm.
Lifting of the face and neck is being revolutionized by a new impalpable, non-absorbable, elastic surgical thread mounted on an atraumatic two-tipped needle; this thread does not cut into the tissues and is transformed into a ligament. In our study, which is aimed at achieving permanent rejuvenation of the patient’s appearance, we have replaced the ample dissections and manipulations of SMAS with the implantation of new suspensive ligaments. These exert traction and transpose tissue volumes exactly where necessary, close to the defect; skin dissection is either minimal or no longer needed.
The procedure often requires correction of both excessive mandibular volume and drooping of the cheeks. This latter is corrected by means of elastic MACS lifting, with an incision that stops at the tragus, or, as in this case, by implanting an elastic suspension thread (Elasticum, Korpo) through an incision of a few millimetres.
A micro-incision made with a n° 11 blade enables the excess subcutaneous fat to be removed by means of liposuction through a 2 mm cannula. Liposuction is carried out in the same direction as the traction.
Once liposuction has been completed, the incision through which the elastic thread will be implanted in order to lift the cheek is made.
